Анотація

Актуальність. Вроджена діафрагмальна кила є складною вадою розвитку з високою анатомічною та клінічною варіабельністю. Пренатальні ультразвукові дослідження дозволяють оцінити тяжкість патологічного процесу у конкретному випадку для визначення тактики ведення вагітності, пологів та неонатального періоду. Проте рідкість даної патології та особливості ультразвукової візуалізації можуть призводити до низької інформативності досліджень. Мета роботи – представити детальну методологію пренатальних ультразвукових досліджень при вродженій діафрагмальній килі з урахуванням анатомічних варіантів патології та особливостей візуалізації в різні терміни вагітності, охарактеризувати передумови діагностичних помилок. Матеріали та методи. Систематизовано дані пренатальних ультразвукових досліджень 265 вагітних жінок групи високого ризику з діафрагмальною килою у плода за період 2007–2020 рр., проведених у відділенні медицини плода Державної установи «Інститут педіатрії, акушерства і гінекології імені академіка О.М. Лук’янової Національної академії медичних наук України». Ультразвукові дослідження виконувались на системах HDI 4000, ACCUVIX V20EX-EXP, ACCUVIX V10LV-EX. Ізольовану ваду виявлено в 192 (72,5 %) випадках, у складі множинних вроджених вад розвитку – у 65 (24,5 %), у поєднанні з хромосомною патологією – у 8 (3,0 %). Лівобічна кила діагностована у 85,7 % спостережень (n = 227), правобічна у 12,1 % (n = 32), центральна в 1,1 % (n = 3), двобічна в 1,1 % (n = 3). Визначені особливості взаємного розташування органів черевної порожнини, заочеревинного простору, грудної клітки та специфіку їх ультразвукової візуалізації в різні терміни вагітності. Результати та їх обговорення. Подано методологію пренатального ультразвукового обстеження плодів із вродженою діафрагмальною килою, розроблену з урахуванням специфіки патологічної анатомії та ультразвукової семіотики. Охарактеризовано типові та рідкісні варіанти локалізації органів грудної клітки та черевної порожнини у стандартному поперечному зрізі на рівні клапанів серця, визначено передумови та варіанти діагностичних помилок. Наведено способи коректного вимірювання грудної клітки, легень, печінки та розрахунку прогностичних індексів для визначення ступеня гіпоплазії легень та ступеня герніації печінки. Висновки. Пренатальна ультразвукова семіотика вроджених діафрагмальних кил характеризується значною варіабельністю, і містить суттєві ризики діагностичних помилок. Подана методологія уніфікованих ультразвукових досліджень дозволяє проводити системну оцінку, отримати коректний пренатальний діагноз із визначенням ступеня тяжкості патологічного процесу і прогнозу для вибору тактики ведення вагітності та пологів.
