Анотація

Актуальність. Синусити, як верхньощелепні, так і інших пазух, є мультидисциплінарною проблемою, з якою постійно стикаються лікарі-отоларингологи, стоматологи, неврологи, офтальмологи. Існує неабияка діагностична цінність антропометричних показників верхньощелепних пазух осіб різної статі і зрілого віку за допомогою КТ-сканування. Мета роботи – встановити взаємозв’язок між формою верхньощелепних пазух та можливими контактами коренів зубів верхньощелепного зубного ряду (ВЩЗР) у здорових чоловіків та жінок зрілого віку. Матеріали та методи. Дослідження виконано за результатами опрацювання 102 конусно-променевих комп’ютерних томограм з варіантами норми верхньощеплених пазух (ВЩП) у осіб зрілого віку у фронтальній та сагітальній проекції на конуснопроменевому томографі Point 3D Combi 500. Дослідження належить до категорії «опис серії випадків» – вид дослідження, що визнається доказовою медициною і не претендує на статистичну значущість результатів. Результати та їх обговорення. При розгляді комп’ютерних томограм чоловіків та жінок досліджуваної групи було з’ясовано вплив форми верхньощелепних пазух на контакт коренів зубів із кортикальною пластинкою та слизовою оболонкою верхньощелепних пазух. Визначено шість основних форм ВЩП: трапецієвидна, квадратна, кругла, овальна, прямокутна та трикутна. У фронтальній проекції КПКТ найчастіший контакт з кортикальною пластинкою ВЩП спостерігаємо при трикутній та прямокутній формі ВЩП, а саме: при прямокутній формі ВЩП контактує з 15, 25-м коренем зуба у 13 осіб (56,5% випадків) та при трикутній формі ВЩП контактує з 15, 25-м коренем зуба у 17 осіб (56,5% випадків). У сагітальній проекції найчастіший контакт з кортикальною пластинкою лівої ВЩП спостерігаємо при трикутній формі ВЩП, а саме – пазуха контактує з 25-м коренем зуба у 38 осіб (59,4% випадків). Найрідше у сагітальній проекції КПКТ спостерігався контакт із коренями зубів при квадратній та круглій формі ВЩП. При квадратній формі ВЩП 13-й та 14-й корені зубів взагалі не контактують з її кортикальною пластинкою, а зі слизовою оболонкою відсутній контакт усіх коренів зубів крім 16-го, при чому лише у 1-ї особи. Висновки. Результати проведених нами досліджень дозволили простежити низку закономірностей впливу форми верхньощелепної пазухи на контактування коренів зубів із нею. Контакт кортикальної пластинки ВЩП найчастіше спостерігається з 16-м та 26-м коренями зубів, а слизової оболонки ВЩП – з 17-м та 27-м коренями зубів верхньощелепного зубного ряду. Встановлено, що при будь-якій формі пазухи у фронтальній проекції не було виявлено жодного контакту коренів 13-го, 23-го і 24-го зубів ВЩЗР зі слизовою оболонкою верхньощелепної пазухи. У фронтальній проекції КПКТ було виявлено, що найчастіший контакт з кортикальною пластинкою ВЩП спостерігаємо при трикутній, трапецієвидній та прямокутній формі ВЩП, а найрідший – при овальній, квадратній та круглій формі ВЩП. Вдалось встановити, що у сагітальній проекції КПКТ найчастіший контакт з кортикальною пластинкою ВЩП спостерігаємо при трикутній формі ВЩП, а найрідший – при круглій.
