Анотація

Актуальність. Захворювання щитоподібної залози є одними з найпоширеніших патологій у світі, зокрема гіпотиреоз, гіпертиреоз і рак щитоподібної залози. На сучасному етапі відзначається збільшення їх випадків. Щитоподібна залоза є надзвичайно важливим елементом для загального здоров’я людини, гормони якої необхідні для нормального функціонування всіх тканин в організмі.

Мета роботи. Систематизувати та узагальнити дані наукових досліджень стосовно значення гормонів ЩЗ (Т4, Т3 і rT3) у розвитку гіпотиреозу, їх впливу на інгібування росту ракових клітин і цитотоксичних ефектів цих гормонів на клітини РЩЗ.

Матеріали та методи. Повнотекстові публікації, які були отримані внаслідок літературного пошуку у вітчизняних (е-каталог Національної бібліотеки України ім. В. І. Вернадського) і закордонних базах даних (Scopus, Web of Science Core Collection, PubMed) за період 2000–2020 рр.

Результати та їх обговорення. Незважаючи на ретельну вивченість функціонування ЩЗ, питання використання тиреоїдних гормонів і лабораторного моніторингу при лікуванні різної патології ЩЗ, до сьогодні залишається дискутабельним. Зокрема це стосується використання комбінації ЛТ4 + ЛТ3 та ролі реверсивного Т3. Це обумовлено складною системою регуляції, яка залежить від багатьох чинників: гормонального гомеостазу, харчових чинників, фізіологічних умов та інше, які спільно підтримують нормальне функціонування ЩЗ. Монотерапія LT4 залишається стандартним методом замісної терапії, проте, згідно з рекомендаціями ЕТА, для окремих пацієнтів можливим є призначення комбінованої терапії LT4 + LT3 як експериментального підходу. Обговорюється питання використання тесту rT3 як прогностичного/ діагностичного індикатора для моніторингу терапії пацієнтів, які отримують терапію L-тироксином. Застосування L-тироксину як тиреотропної супресивної терапії при лікуванні раку щитоподібної залози вимагає ретельного коригування дози в залежності від стану здоров’я кожного пацієнта, ризику рецидиву, що вимагає моніторингу та динамічної переоцінки. Довгострокова супресивна терапія LT4 може впливати на обмін речовин, сприяючи зниженню маси тіла.

Висновки. З огляду на аналіз даних літератури, який охопив 20 років, на сьогодні монотерапія L-Т4 є стандартною терапією хворих із гіпотиреозом. Комбіновану терапію LT4 + LT3 можна рекомендувати пацієнтам, які отримують лікування L-T4 зі скаргами, що характерні для гіпотиреозу, незважаючи на нормальні рівні ТТГ, за винятком інших хронічних та супутніх аутоімунних захворювань, які можуть бути їх причиною.
